Case Manager
The Next Door – Knoxville
Job Title: Case Manager, TND-Knoxville
Reports To: Program Director, TND-Knoxville
Mission: The Next Door’s mission is to meet the physical, emotional and spiritual needs of women in crisis, equipping them to develop lives of wholeness and hope.
Summary: To provide intensive case management services for women who are leaving incarceration to become residents of The Next Door.
Primary Responsibilities and Duties include the following:
- Work with residents to create an individualized case management service plan.
- Assess each resident’s progress bi-weekly; determining goals and areas that need additional attention.
- Teach and model effective problem-solving skills using cognitive behavioral approaches.
- Ensure timely and professional documentation to support each resident’s interaction and coordination of services.
- Connect program participants to community resources to address their comprehensive needs.
- Provide employment support to include resume writing and job retention skills.
- Assess each resident to ensure that her daily medical, psychosocial and environmental needs are met.
- Coordinate with parole and probation officers to ensure proper coordination of services on behalf of residents.
- Coordinate meetings/visits with residents and their families and develop and maintain relationships with the family unit to support successful communication and transition back into the community.
- Create a discharge plan with residents upon their completion of The Next Door program which includes connections and appointments for ongoing support services in the area which the residents choose to re-locate.
- Conduct exit interviews when possible.
- Assist in the admissions process; building relationships with drug courts, correctional community, etc.
- Ensure that all policies and procedures of TND- Correctional Release Center are followed at all times by residents, volunteers, and employees.
- Participate in on-call rotation for after hours as directed by supervisor.
- Model ethical and professional behavior at all times.
Qualifications:
To be successful in executing the responsibilities and duties assigned in this job, a person must be able to perform each essential function satisfactorily. This person must be self-motivated and able to make quality, ethical decisions independent of supervision when necessary. This person must be willing to abide by the Statement of Faith for The Next Door.

Probation period: 90 days
Education and Experience:
Bachelors or masters degree in social work or related field required. Experience working with women in crisis situations preferred. Working knowledge of Knoxville social service community preferred.
Other Requirements:
1. Prioritize and manage multiple projects simultaneously.
2. Possess computer skills including e-mail, Word, and Excel.
3. Valid Tennessee driver’s license.
4. Personal automobile insurance.
